If you were hurt in the

Choice of Treating
Doctors – While your employer can send you to a doctor (or doctors) for
the purpose of your undergoing an “independent medical examination” (IME), your
employer CANNOT order you to be treated (or receive treatment)
from any particular doctor or medical facility. You are entitled to receive treatment
by two (2) doctors of your own choosing, as well as, to receive treatment from doctors
(or other medical providers) to whom you were referred by either of your first two
doctors;

Weekly Lost Wage
Benefit – If your doctor feels it is necessary for you to be off work to
recuperate from your injuries and/or to wait until a surgery and/or other medical
procedure can be performed on you, you are entitled to receive Temporary Total
Disability (TTD) benefits for the time that you are totally unable to work. TTD is a
weekly wage benefit which is based on two-thirds of the average, gross, weekly wages
received by you from this employer for the time period extending 52 weeks prior to the
date of your accident. TTD is not subject to income tax;
What you must do to protect and reserve your rights to Workers’ Compensation
benefits:
As a practical matter, you
should give that notice as soon as possible after the accident.
Notice can be given orally or
in writing, but it is recommended you give notice in writing; and if possible, by filling out an
Accident Report Form and getting some supervisor to sign and date your copy of the
notice.
Never sign a blank
Accident Report Form; and always keep a copy of the completed accident report for your
records.
